Medical Case Manager

Reports To: Ryan White Program Director

FLSA Status: Non-Exempt

Job Summary: The Ryan White Medical Case Manager (MCM) provides a full range of medical and supportive case management services to People Living with HIV (PLWH) including those who have complex medical and social needs, have multiple diagnoses, are living with a disability, are unstably housed, or have other significant barriers to care. In collaboration with other members of the healthcare team, the MCM links consumers to medical and supportive services by organizing, coordinating, and providing case management services to consumers within the Ryan White Program. The MCM will assist with accessing medical, social, community, or any other resources under the Ryan White consortium. The MCM will protect the dignity, privacy and confidentiality of patients and their families; and work collaboratively with other Daily Planet providers and staff to promote a team-based approach to patient care.

Job Requirements: Bachelor’s degree in social work, public health, or other health and human services related field or a licensed RN or 2 years of other Ryan White related experience. (1) to three (3) years of Case Management experience, knowledge of Ryan White Part B Service Standards (Preferred), knowledge of HIV disease and knowledge in chemical dependency, mental health, housing, and homeless service systems. Demonstrate strong critical thinking, analytical and problem-solving skills. Candidate must have skills including excellent oral and written communication, organizational skills, and Microsoft office. Must demonstrate skill and knowledge in client assessment, engagement, and group exercises. Must demonstrate attention to detail. Must be able to meet deadlines for all reports.
