Description:

The Southwest Center for HIV/AIDS is looking for a compassionate Medical Case Manager to provide various levels of case management assistance to individuals living with HIV/AIDS. Many of the clients seek help with medication adherence, chemical dependency, mental illness, housing instability, food insecurity, and other issues. Our case management team helps people obtain and sustain the medical care and other core and support services they need to live long, healthy lives. This person is someone who truly wants to make a difference in the community that we are dedicated to serve.


The Medical Case Manager supports the Southwest Center mission of providing affirming and inclusive services to promote well-being and advance health equity for diverse communities and all those seeking compassionate care - especially people of color, LGBTQIA2S+ and Queer individuals, and those affected by HIV.


What your day will look like at Southwest Center:

  • Conduct comprehensive assessments (psycho-social, program/service eligibility) and re-assessments of case management clients.
  • Complete individual service plans with clients to set goals and strategies to overcome barriers (including but not limited to mental health, chemical health, housing instability or criminal history) to maintaining medical care.
  • Timely and coordinated access to medically appropriate levels of health and supportive services and continuity of care.
  • Continuous client monitoring to assess the efficacy of the care plan. Re-evaluation of the care plan every six months with adaptations as necessary.
  • Treatment adherence counseling to ensure readiness for and adherence to complex HIV treatments.
  • Maintain client records and files, collect required documentation from clients, complete data entry of client notes, process information for program reports and invoicing, etc.in a prompt manner.
  • Participate in team wide activities and initiatives including trainings and meetings.
  • Engage in outreach and recruitment of new clients at membership services activities throughout the year.

Requirements:

Who we're looking for:

  • You've earned a Bachelor's degree in social work or comparable health or human services field, OR you have a minimum of 4 years of case management experience.
  • You have knowledge of social work and case management guidelines and issues related to working with low-income clients, including data privacy, confidentiality, and HIPAA compliance.
  • You have knowledge of issues and community resources related to HIV/AIDS, chemical dependency, mental health, housing, health insurance, and other barriers.
  • You have the ability to listen well and assist clients in de-escalation and problem solving.
  • You have experience working with communities of color and/or working with Gay/Bi/MSM/Queer and Transgender health issues. You understand the complex issues our marginalized populations are faced with and the barriers they experience accessing healthcare.
  • You have knowledge of and skills using personal computer equipment and related software for word processing (MS Word), spreadsheets (MS Excel) and CareWare.
  • You have demonstrated experience preparing client files, records, and program reports.
  • You have strong and timely communication skills with both clients and other professionals.
  • You work with clients respectfully and act in a confidential manner.
  • You can manage, prioritize, categorize, and complete multiple tasks.
  • You can manage stress in a proactive manner.
  • You have demonstrated cultural competency to serve + the LGBTQIAS professionally and respectfully communities.

Environmental Factors, Physical Requirements, and Compliance

  • Work primarily in a climate-controlled environment with minimal safety/health hazard potential.
  • Office environment, exposure to computer screens for lengthy periods of time.
  • The position requires extensive use of computers using a standard computer keyboard.
  • This position may require evening and occasional weekend work.
  • While performing the duties of this job, the employee is regularly required to walk, sit, stand; use hands, talk, and hear. The employee is occasionally required to reach with hands and arms. Specific vision abilities required by this job include close vision, distance vision, and ability to adjust focus.
  • Currently have, or be able to obtain within 90 days of employment, a valid Fingerprint Clearance Card
  • Currently have, or are able to obtain within 7 days of employment, a clear TB test
  • Currently have, or are able to obtain within 7 days of employment, a current flu vaccination
  • Currently have, or are able to initiate within 7 days of employment, a Hepatitis-B primary vaccination series
  • Currently have, or are able to obtain within 14 days of employment, a CPR certification
