Job Description
Responsible for ensuring the optimal facilitation of medical care for the homeless clients that are discharged from hospitals or other agencies contracted with Recuperative Care Centers.
- Complete initial medical assessments of client upon arrival which includes: head to toe assessment, checking vitals, patient education, determine insurance status, make medical follow-up appointments with clients’ doctors
- Input and update client’s information and medical notes in KIPU EMR system
- Conduct rounds by reminding clients of medication, assisting client with checking BSL, and assisting with minor wound care teaching if necessary
- Ensure the success of a client’s hospital discharge by confirming hospital instructions and checking medication
- Promote health literacy by providing patient education with regards to medical, treatment management, diet, and personal health care procedures
- Collaborate with Medical Coordinators and Health Care Providers to ensure best patient outcome
- Serve as an advocate for client to ensure best medical care is provided by linking clients to medical resources and MediCare or Medi-Cal as needed
Minimum Qualifications/Preferred Experience:
- Current and non-restricted MA or EMT certification or license/diploma
- Experience working with the homeless or geriatric population (preferred)
