Somerfield Health Center at The Heritage at Brentwood is currently seeking an exempt, full-time MDS Coordinator/RN for overall supervision, coordination, maintenance, and timely completion of Material Data Set (MDS), care plans and medical records for nursing care services in our 5-star, 66-bed skilled nursing facility on the beautiful campus of The Heritage at Brentwood in TN. Job Responsibilities

• Coordinates, conducts, and completes MDS, resident care plans, and conference on the electronic medical records system while ensuring accuracy and completeness of entire medical record
• Ensures and monitors that MDS are completed within HCFA and Medicare guidelines and submits electronica MDS per state regulations
• Participates in family/resident care plan conferences
• Performs as staff nurse when staffing levels necessitate, including passing medication cart and responding to emergency calls
• Assists Director of Nursing in developing and implementing medical records policies and procedures
• Assures timeliness of physician visits and notifies physician and family of changes in residents' conditions

Qualifications

• Active, unrestricted RN license in Tennessee required
• Nursing supervisory experience preferred
• Working knowledge of MDS process and regulations impacting MDS
• At least 6 months' experience working in long-term care setting preferred
• Geriatric/rehabilitation experience with good understanding of restorative nursing preferred
• Able to provide emotional, physical, and psychological support to senior residents
• Ability to effective lead a team and to handle multiple tasks safely and effectively
• Excellent communication skills, including the ability to communicate sensitive information with empathy
• Ability to work effectively with frequent interruptions and attention to detail
