Overview:
We are seeking a highly organized and detail-oriented MDS Coordinator to join our team. As an MDS Coordinator, you will play a crucial role in ensuring accurate and comprehensive documentation for our patients. Your expertise in case management, primary care, and medical coding will be essential in providing quality patient care and facilitating effective discharge planning.
Responsibilities:
- Conduct comprehensive assessments of patients' medical conditions and care needs
- Review and analyze medical records, documentation, and patient charts
- Collaborate with healthcare professionals to develop individualized care plans
- Ensure compliance with regulatory guidelines and documentation requirements
- Perform utilization reviews to optimize patient outcomes and resource allocation
- Utilize ICD-9, DRG, and ICD-10 codes to accurately code diagnoses and procedures
- Coordinate with interdisciplinary teams to facilitate seamless transitions of care
- Monitor patient progress and update care plans as needed
- Provide education and training to staff on proper documentation practices
Experience:
- Previous experience in case management or primary care setting
- Strong knowledge of medical coding principles and practices
- Familiarity with utilization review processes and discharge planning
- Proficiency in ICD-9, DRG, and ICD-10 coding systems
- Excellent attention to detail and ability to maintain accurate records
- Strong communication skills to collaborate effectively with healthcare professionals
- Ability to prioritize tasks and manage time efficiently
