The Manager of Social Work Services for Affirm: The RUSH Center for Gender, Sexuality and Reproductive Health is an integral member of the RUSH Office of Community Health Equity and Engagement who focuses on services and programming for the LGBTQ community internal at RUSH and within the communities served by RUSH. This position is responsible for the delivery and management of Social Work and patient navigation services and serves as the direct manager of a team of patient navigators. This position collaborates closely with other members of the Affirm team on institutional and grant-related deliverables as they pertain to services and care for LGBTQ populations at RUSH.

The Manager also collaborates closely with RUSH's Gender Affirmation Surgical Clinic and other inter-professional healthcare teams within the RUSH system for the growth and sustainability of clinical services. The Manager of Social Work Services works collaboratively with others from Affirm to build community-facing programs and advance RUSH's mission to bring health equity to LGBTQ populations. Finally, this position exemplifies the RUSH mission, vision and values, and acts in accordance with RUSH policies and procedures..Position Responsibilities: Participates in development, implementation, and evaluation of innovative programming related to gender, sexuality and reproductive health Services for LGBTQ populations.Builds and manages workflows for emerging services.Assists Directors in the implementation of regulatory standards and quality improvement efforts as they pertain to the provision of health services.Participates in the continuous quality improvement process within clinic, RUMG and Rush University Medical Center.Assists with identification of health outcomes to monitor effectiveness of programming, services, and patient outcomes.Participates in quality improvement activities including data collection, chart review, inter-professional collaboration, analysis of patient data, and care team meetings.Provides direct Social Work ServicesCarries out comprehensive Social Work assessments by phone or in-person to identify patients in need of Social Work and behavioral health services.Performs and documents comprehensive psychosocial assessments.Develops and documents evidence-based individualized care for identified patients, focusing on specific psychosocial goals.Develops evidence-based plans of care that address barriers with the patient, patient¡¦s family, and care team member to achieving mental health and function.Provides psychosocial education, counseling, anticipatory guidance, and linkage to community resources during both individual and group sessions for patients and families as indicated by comprehensive assessment.Advocates within complex medical and community-based service systems to improve patient access to needed services.

Maintains relationships with referring Rush clinics through consistent and timely communication.Provides educational materials and resources to patient/family that are in a format which is specific to the patient/family learning abilities and language preference.Effectively collaborates with inter-professional healthcare teams within and outside of the Rush system.Responsible for the management of patient navigation and social services as they pertain to gender, sexuality, and reproductive health servicesServes as content expert for Rush Social Workers.Provides direct supervision for Patient Navigators and case management services.Responsible for overseeing Patient Navigators¡¦ performance evaluations, vacation requests and time and attendance issues.Participates in policy and procedure development and review for clinical activities.Participates in recruitment efforts, interviewing, and hiring of staff.Responsible for orientation of new staff.Coordinates staff schedules ensuring adequate coverage of required activities.Fosters a climate that encourages positive staff morale, motivation, and commitment.Applies interpersonal communication skills to maintain an open network of communication among staff members and other health care professionals.Responsible for maintenance of standards of care mandated by regulatory agencies.Assumes responsibility for maintaining competence in own professional practice and maintenance of required credentials.Attends RUMC individual and group supervision meetings for outpatient Social Workers as well as relevant Social Work trainings.Maintains the standards and educational requirements specific to the position through continuing education.Maintains and updates knowledge and skills based on current practice and research.Updates knowledge of current developments in behavioral health management, health care legislature, and hospital/community resources.Performs related duties as required.Position Qualifications Include: Master's Degree in Social Work; LCSW required. 5 year of experience preferably in community setting.Clinical experience with LGBTQ populations.Three years management or supervisory experience preferred.Excellent written and verbal communication skills. Computer knowledge (Excel, Word, PowerPoint is a plus)Excellent negotiation, delegation, time management and problem solving skills.Strong assessment and intervention skills with a diverse population.Excellent understanding of and ability to implement trauma-informed practices.Demonstrated ability to triage and manage own work.Demonstrates customer-focused service skills; patient centered, problem solving advocate.Demonstrated ability to work on an interprofessional team, enhancing team function and communication.Company Highlights:Ranked among the top academic medical centers in the country, Rush University Medical Center is an outstanding place to enhance your career.
