The role of the Manager is to ensure that the mission and strategic goals of St. Joseph Health: St. Joseph and Redwood Memorial Hospitals is supported by efficient and effective patient flow across the continuum of care. The Manager will provide direct supervision to caregivers in the Care Management. The Manager provides collaborative support and expertise on complex case management and service coordination; functions as a clinical resource and mentor department staff in promoting quality care and integration of services across the health care continuum. The Manager will assure proper use funds, will represent organization at meetings and will provide appropriate required documentation.

Required qualifications:
- Bachelor's Degree in Healthcare related field. Or equivalent education/experience.
- California Registered Nurse License upon hire.
- 3 years Supervisory or management experience in an acute care setting.
- Recent experience in providing direct client services, case management, program development and management of performance management activities.
Preferred qualifications:
- Master's Degree in Healthcare related field or Social Work.
- Clinical acute care experience.
