Managed Care Community Connector Bilingualin Spanish or Bengali (Remote – NYC)
We are looking to hire a candidate with the experience and skill sets mentioned for one of our major clients within the Health Insurance industry. This is a REMOTE role. This is a 6-month contract opportunity. There is a possibility that the position will be temp to perm employment based on positive work. The position is fully remote, but candidates MUST be local to New York City (Brooklyn, Queens, Bronx, or Manhattan). The schedule will be Monday to Friday, 8:30 am – 5 pm EST.
*Bilingual with Spanish and/or Bengali.
Position Summary:
Community Connectors works with members, providers, and multidisciplinary team members to assess, facilitate, plan, and coordinate an integrated delivery of care across the continuum, including behavioral health and long-term care, for members with high need potential. Staff work to ensure that patients progress toward desired outcomes with quality care that is medically appropriate and cost-effective based on the severity of illness and the site of service.
Position Responsibilities:
- Contact members with Health Conditions.
- Serves as a community-based member advocate and resource, using knowledge of the community and resources available to engage and assist vulnerable members in managing their healthcare needs.
- Collaborates with and supports the Healthcare Services team by providing non-clinical paraprofessional duties in the field, including meeting with members in their homes, nursing homes, shelters, or doctor's offices, etc.
- Empower members by helping them navigate and maximize their health plan benefits. Assistance may include:
- Scheduling appointments with providers.
- Arranging transportation for healthcare visits.
- Getting prescriptions filled.
- Following up with members on missed appointments.
- Assists members in accessing social services such as community-based resources for housing, food, employment, etc.
- Provides outreach to locate and/or provide support for disconnected members with special needs.
- Conducts research with available data to locate members the company has been unable to contact (e.g., reviewing internal databases, contacting member providers or caregivers, or traveling to the last known address or community resource locations such as homeless shelters, etc.).
- Participates in ongoing or project-based activities that may require extensive member outreach (telephonically and/or face-to-face).
- Guides members to maintain Medicaid eligibility and with other financial resources as appropriate.
- 50-80% local travel may be required. Reliable transportation is required.
Required Skills/Experience/Education:
- 2+ years of experience working with underserved or special needs populations, with varied health, economic, and educational circumstances.
- Candidate MUST have customer service skills and be empathetic as there will be a lot of contact/communication with members.
- Candidate must be Bilingual in English/Spanish or English/ Bengali.
- Must have a valid driver's license with a good driving record and be able to drive within the applicable state or locality with reliable transportation.
- HS Diploma/GED, Associate’s or Bachelor’s degree.
Other job specifications:
- Employment Type: Contract to Hire (CTH), W2 only. NO C2C.
- Contracting Period: 6-month contracting opportunity with the possibility for extension.
- Job Location: The position will require traveling and visiting members. It is fully remote, but candidates MUST be local to New York City (Brooklyn, Queens, Bronx, or Manhattan). 50-80% local travel may be required. Reliable transportation is required.
- The Schedule: Monday to Friday 8:30 am – 5:00 pm EST.
- Contracting Rate: $22/hr. on W2.
- Interview Process: Microsoft Teams Meeting (1 interview).
