Lvn Service Coordinator/Case Manager

**Remote but Must reside in or around the Uvalde or Del Rio, TX area**

LVN Service Coordinator Compensation:

• The pay range for this position is$22-$26 per hour depending on experience
• Full benefits available after 90 days: Medical, Dental, Vision, Life, Short-term Disability
• GREAT Hours! Monday through Friday, 40 hours per week, No Weekends

LVN Service Coordinator Highlights:

• The required availability is M-F 8am-5pm
• Assessments will take place via home visits in the Del Rio/Uvalde TX area
• 3 days of visits and 2 days remote

LVN Service Coordinator Job Responsibilities:

• SC will have 3 days of visits and 2 days of documentation weekly
• SC will have a caseload and work assigned tasks
• SC will complete H2060 assessment
• SC will fill put required forms for each assessment
• SC will make follow up calls

Describe the performance expectations/metrics for this individual and their team:

• SC will complete documentation within 3 day turnaround time
• SC will stay organized and ensure work they do not fall behind
• SC will maintain a 90% or high in Audit scores

LVN Service Coordinator Requirements/Qualifications:

• 2+ years of experience in a managed care environment or working with people with disabilities and vulnerable populations who have chronic or complex conditions.
• LVN License active and in good standing OR Bachelor’s degree in Social Work, Health Services, Behavioral Science or related field, OR equivalent experience
• Drivers License

Preferred Experience/Skills:

• Service Coordination, Case Management or Nursing experience preferred

What previous job titles or background work well in this role?

• 2+ years of experience in a managed care environment or working with people with disabilities and vulnerable populations who have chronic or complex conditions
• LVN’s with home health experience
