Lvn Scheduler - Theracare Home Health Houston

Job Summary:

The Home Healthcare Scheduler is responsible for maintaining accurate scheduling for clients and staff for multiple disciplines.

Qualifications:

· High School Graduate (college preferred)

· Licensed LVN, a plus but not required

· Prior scheduling experience preferred with multiple disciplines

· Good computer skills including but not limited to MS Office, MS Excel, Outlook

· Experience using HomeCare HomeBase preferred

· Knowledge of basic medical terminology

· Ability to articulate interpretation of reports

· Excellent customer services skills, both internal and external

· Well organized with the proven ability to be efficient while multi-taskin

Essential Functions:

· Understands and adheres to established TheraCare policies and procedures

· Timely and professional/courteous communication with all team members

· Responds timely to email/phone calls

· Maintains accurate scheduling for clients and staff for multiple disciplines

· Contacts care providers and clients regarding day-to-day changes

· Provides staffing for sick leave, vacation, long term leave

· Maintains staff and client database

· First line in visit utilization, missed visits and employee productivity

· Discerns client services required as outlined in agreements, urgent requests and care plans

· Assigns case managers and clinical teams according to territory

· Completes/clears assigned workflow daily and accurately

· Sends contractor referrals timely upon approval from administrator/DON

· Participates as needed in case conferences

· Daily Productivity reports as dictated by administrator
