JOB DESCRIPTION SUMMARY
JOB RESPONSIBILITIES
Patient Care
1. Conducting home visits as assigned, assessing the patient's condition, progress toward goals, providing patient care, teaching and other skilled interventions as ordered.
2. Regularly re-evaluating and reporting patient nursing needs, as part of nursing visit.
3. Providing health care instructions to the patient as appropriate per assessment and plan of care.
4. Identifying discharge planning needs during nursing visits. Communicating needs to team as part of the care plan development; assists in implementing prior to discharge of the patient.
5. Assuring proper maintenance of clinical records in compliance with local, state and federal laws.
Administrative Responsibilities
1. Conduct the initial visit with the patient within 24 hours from the moment patient case was accepted.
2. Communicate any scheduling difficulties with the office immediately preventing any patient care delay.
3. Complete comprehensive care as ordered, report any findings in a timely fashion.
4. Complete and turn in all required paperwork and online charting within 72 hours from the day the visit was conducted.
5. Comply with the basic processes and regulations of the company.
Qualifications and Skills
1. Current CA LVN license required.
2. 1+ year experience in Home Health
3. Health Care Provider CPR Certification
4. Excellent observation, verbal and written communication skills, problem solving skills, nursing skills per competency checklist.
5. Ability to travel.
6. Experience working with an EMR system (DSL (preferred), SYNERGY, KINSER)
Job Types: Contract, Per diem, Travel nursing
Pay: $40.00 - $50.00 per hour
Expected hours: No less than 30 per week
