LPN - Home Health

Duties:
- Provide skilled nursing care to patients in their homes
- Administer medications and treatments as prescribed by physicians
- Perform wound care, including suture removal and dressing changes
- Provide specialized care for pediatric patients
- Utilize electronic medical record system (eClinicalWorks) to document patient care and treatment
- Assist patients with activities of daily living, including lifting and transferring
- Manage airways and provide respiratory support as needed
- Collaborate with interdisciplinary healthcare team to coordinate discharge planning
- Stay up-to-date with current nursing practices and attend continuing education programs

Experience:
- Valid LPN license in the state of NJ
- Minimum 1 year of experience in home health or a related field
- Experience with medication administration and management
- Knowledge of Geriatric/pediatric nursing principles and best practices
- Familiarity with electronic medical record systems, such as eClinicalWorks
- Ability to lift and transfer patients safely and effectively
- Competency in airway management techniques
- Understanding of discharge planning processes and procedures
