Licensed Therapist

The Licensed Therapist is a core member of the care team, including the mental health navigator (MHN) and psychiatric consultant. The Licensed Therapist is responsible for delivering evidence-based psychotherapy to clients on an assigned caseload and coordinating with the treatment team, and when appropriate, other care providers.

DUTIES AND RESPONSIBILITIES

1. Support the mental health care of clients on an assigned patient caseload. Closely coordinate care with the treatment team, when appropriate, other mental health providers.
2. Screen and assess clients for common mental health and substance abuse disorders. Facilitate patient engagement and follow-up care.
3. Provide patient education about common mental health and substance abuse disorders and engage in shared-decision making to determine a treatment plan.
4. Systematically track treatment response and monitor clients (before or during sessions) for changes in clinical symptoms.
5. Consult with other treatment team members when concerns arise about treatment engagement or progress through treatment.
6. Provide psychotherapy using evidence-based techniques such as cognitive behavioral therapy, acceptance and commitment therapy, interpersonal therapy, or other treatments as appropriate.
7. Provide or facilitate outside referrals to a higher level of care as clinically indicated.
8. Participate in regularly scheduled (usually weekly) caseload consultation with the psychiatric consultant and mental health navigator. Consultations will focus on clients new to the caseload and those who are not improving as expected under the current treatment plan. Case reviews may be conducted by telephone, video, or in person.
9. Track patient follow up and clinical outcomes using a registry. Document all patient encounters in the registry and use the system to identify clients in need of outreach or a change to their treatment plan.
10. Document patient progress and treatment recommendations in EHR and other required systems so as to be shared with psychiatric consultant, mental health navigator, and other necessary team members.
11. Facilitate treatment plan changes for clients who are not improving as expected in consultation with the psychiatric consultant and who may need more intensive or more specialized mental health care.

1. Alert Mental Health Navigator (MHN) to need for referrals for services outside of the organization (e.g., social services such as housing assistance, vocational rehabilitation, resources for food insecurity).
2. Develop and complete relapse prevention self-management plan with clients who have achieved their treatment goals and are soon to be discharged from the caseload.

REQUIRED SKILLS

•  Demonstrated ability to collaborate and communicate effectively in a team setting.
•  Ability to maintain effective and professional relationships with clients and other members of the care team.
•  Experience with screening for common mental health and substance abuse disorders.
•  Working knowledge of differential diagnosis of common mental health and substance abuse disorders.
•  Ability to effectively engage clients in a therapeutic relationship.
• Ability to work with clients by video as well as by telephone, if needed.
•  Experience with assessment and treatment planning for common mental health and substance use disorders.
•  Working knowledge of evidence-based psychosocial treatments and behavioral
  interventions for common mental health disorders, when appropriate (e.g., motivational
  interviewing, problem-solving treatment, behavioral activation).
•  Experience delivering evidence-based psychotherapy techniques.

TRAINING & LICENSURE REQUIREMENTS

Therapist must have one of the below-listed credentials and be licensed in the state of Texas to perform the duties and responsibilities of the position.

•  Licensed Mental Health Counselor
•  Licensed Professional Counselor
•  Licensed Marriage and Family Therapist
•  Licensed Clinical Social Worker
•  Licensed Psychologist  (PhD or PsyD)
•  Masters-level licensure candidate/trainee (e.g. LMSW)
