DUTIES &RESPONSIBILITIES:
- Assist in the planning, developing, organizing,implementing, evaluating, and directing of the Social Services Departmentwithin the facility in accordance with current existing Federal, State andlocal standards
- Develop and manage the initial contact asnecessary with the discharge planner, family or responsible party to initiatetour of facility and follow up procedures
- Assist in developing and implementing policiesand procedures for identifying the medically-related social and emotional needsof the patient/resident
- Develop and maintain a good working rapport withintra-department personnel, other departments within the facility and outsidecommunity health, welfare and social agencies to ensure that social serviceprograms can be properly maintained to meet the needs of the patients/residents
- Keep abreast of current federal and stateregulations as well as professional standards
QUALIFICATIONS &REQUIREMENTS:
- Must have a current, unencumbered Social Work license (as required by state)
- Must be a Licensed Drug and Alcohol Counselor in the state of NM
- Bachelor's Degree in Social Work strongly preferred
- Must have at least 3+ years of experience in a health care facility; preferably a long term care hospital
- Must be able to organize and prioritize manytasks effectively
- Ideal candidate must be a skilled communicator,director and motivator
