Job Description
- Provide outpatient therapy services in the office and in the community
- Develop and maintain Individual Service Plans and Quarterly reports with clients
- Maintain a small caseload of 1-3 supervisees for leadership experience
- Complete comprehensive assessments, service authorizations, and discharge planning
- Case management functions of linking, arranging, and advocating for services and referrals
- Maintain up-to-date client records, and prepare required session notes in a timely and accurate manner
- Work in various departments as needed including Crisis Stabilization, Community Based Services, and Substance Use services, as one’s skill sets allow.
- Perform community outreach and advocacy with other public and private agencies/services
- Collaborate with licensed owner and director during weekly supervision and monthly staff meetings
- Respond promptly to emails and telecommunication.
- Review progress notes, quarterly reports, and ISPs to ensure that clients are working towards goals and objectives and that documentation is clinically sound and aligned with the agency, DBHDS, and Medicaid guidelines
- Work responsibly with the administrative team and direct supervisor
