The Licensed Practical Nurseis responsible for, but not limited to:
- Greets customers in a friendly and welcoming manner. Creates a positive first impression. Smiles, projects a positive attitude.
- Through the continuous improvement of standards of practice and by providing a safe and secure environment the customer will be assured a sense of safety and well-being, free from risk and harm.
- Participates in development of Plan of Care with Registered Nurse and to provide skilled nursing care to patients in need of Home Care services.
- The care provided will meet Agency policies, State and Federal Conditions of Participation and JCAHO Accreditation standards.
- Accurately documents care provided and maintains patient records according to Agency policies, State and Federal requirements and JCAHO standards.
- Assists with Agency Programs. Shares in providing 24 hour 7 days a week coverage to patients.
- Available as needed for assignment to meet the changing needs of patient care 24 hours a day; 7 days a week.
Education:
High School Diploma or equivalent
Professional Experience:
- Current LPN license within the State of New Jersey
- 1 year of Community Health Nursing
