L.P.N. (Licensed Practical Nurse)
32 HRS
$29 - $34.12
REPORTS TO: Charge Nurse and Director of Nursing.
POSITION SUMMARY:
  • Maintains a professional environment in a multi-provider, multidisciplinary organization.
  • Maintains a patient centered environment including but not limited to:
  • Provision of care and advocate for patients of all ages and their families based on prudent nursing and acceptable standards of practice.
  • Efficient patient flow, assisting providers and other staff.
  • Performs tasks and procedures within the scope of a Licensed Practical Nurse (L.P.N.) and legal guidelines of the Nurse Practice Act.
  • Performs nursing tasks in all clinical areas.
QUALIFICATIONS:
  • Graduate of an accredited school of nursing.
  • Current state licensure to practice in Massachusetts.
  • Cardio-pulmonary resuscitation (C.P.R.) certified.
  • Maintains licensure and certifications.
  • Bilingual preferred.

  • Nursing principles including disease process, certain examinations, observations, laboratory studies and appropriately apply these principles to patient care.

PRINCIPAL DUTIES AND RESPONSIBILITIES:
Professional Behavior:

Tasks – including but not limited to:
  • Patient preparation for visit.
  • Appropriately prepare patient for either a complete physical, sick visit, follow up visit; or a gynecological exam.
  • Vital Signs: blood pressure, temperature, pulse (all points), orthostatic blood pressure and pulse.
  • Obtain interpretation when needed.
  • Chart documentation in EHR.
  • Measurements: Height, Weight, and (other measurements as needed).
  • Documentation of reason for visit, medication review, allergy review.
  • Documentation of medication review.
  • Patient tests/treatments.
  • Audio acuity testing.
  • Ear irrigation.
  • Electrocardiogram (EKG).
  • Visual acuity testing.
  • Injections – Intramuscular (I.M.); subcutaneous (S.C), Intradermal (I.D.);
Z-track.
  • Medication administration.
Dose calculations.
  • Peak flow meter.
  • Oxygen saturation.
  • Nebulizer treatment.
  • Oxygen administration.
  • Laboratory Tests
  • CLIA Waived Tests
  • Pap Smear Handling
  • Venipuncture – collect and process specimens.
  • Patient Procedures.
  • Physical and emotional assessment.
  • Wound care.
  • Assisting providers.
  • Maintaining sterile field and sterile technique.
  • Burn care.
  • Diabetic care.
  • Ace wraps/splints/slings/braces.
  • Colposcopy- Assists
  • Biopsies.
  • Suture removal.
  • Staple removal.
  • Participation in the PCMH includes but is not limited to:
  • Review of patient at each visit.
  • Follow-up of Hospital Discharges within forty-eight (48) business hours and involves medical assistants as appropriate
  • Directs and assists medical assistants as needed for patient preventative measure issues.
  • Other Patient Services.
  • Patient education.
  • Prescription refill.
  • Telephone Triage.
  • Walk-in Triage.
  • Other related phone calls.
  • Referrals.
  • Appointments.
  • Nurse visits.
  • Documentation.
  • Interpreting as needed.
  • Serves as chaperone as needed.
  • Care of Equipment.
  • Proper handling, cleaning, and storage of all equipment
  • Other tasks as needed.
QUALITY ASSURANCE/IMPROVEMENT:
  • Quality assurance monitoring – participates in the betterment of the health center through studies and reviews as necessary.
  • Goal setting – assess personal, team, and organization goals.
  • Participates in devising, reviewing, and revising policies, procedures and protocols.
TEAMWORK ORIENTATION:
  • Maintains and encourages teamwork approach.
  • Flexibility – continuously participates in the modification of schedule, duties, and procedural functions.
  • Maintains consistent effort to further goals by modifying and/or improving individual procedures and tasks.
  • Conflict Management – interacts efficiently in conflict management
  • Demonstrates and implements effective problem solving.
  • Identifies tasks and operational problems. Generates solutions, discuss solutions with charge nurse or nurse manager and implements as directed by charge nurse or nurse manager.
  • Manages conflict with staff appropriately
  • Maintains and encourages good communication skills.
  • Gives direction to medical assistants as appropriate.
  • Participates in orientation and training of new staff and/or students.
WORKING CONDITIONS:
Works in a busy community health center in a downtown business district. Long hours are sometimes required.
PHYSICAL REQUIREMENTS:
Physical demands requiring lifting of small pieces of equipment, boxes, and /or patients not to exceed forty (40) pounds. Mostly standing and walking required. Visual acuity sufficient for frequent reading. Hearing acuity sufficient for holding conversation with or without audio devices.

BNHC is committed to providing a safe and heathy environment for patients and staff. As a condition of employment, employees, to include new hires, are required to be fully vaccinated to include the most recent COVID 19 booster and annual flu vaccines, with the exception of those who have documented medical or religious exemptions, that must be approved by BNHC according to its protocols. Additional vaccines may be required based on the recommendation of the CDC. New hires who are not yet vaccinated for flu or COVID can arrange to be vaccinated on site.
