Responsibilities:
- Administer medications and treatments according to physician orders
- Monitor and assess residents' health and promptly report any changes to the nursing and medical team
- Collaborate with the interdisciplinary team to develop and implement individualized care plans
- Assist with the coordination of resident activities of daily living (ADLs)
- Provide emotional support and companionship to residents
- Maintain accurate and up-to-date medical records
Qualifications:
- Valid New York State LPN license
- CPR certification
- Strong communication and interpersonal skills
- Ability to work collaboratively in a team environment
- Experience in a long-term care setting is preferre
