Job Opportunity: Licensed Practical Nurse (LPN)
Reports To: Residential Clinical Supervisor
Position Overview:

We are actively searching for a compassionate and skilled Licensed Practical Nurse (LPN) to join their team. In this role, you will report to the Residential Clinical Supervisor and play a crucial role in providing direct patient services, ensuring their well-being and safety.
Key Responsibilities:
- Provide direct patient services, including basic medical evaluations, first aid, dietary assessments, health education, and vital sign monitoring.
- Collaborate with clinical staff to ensure comprehensive care for clients.
- Monitor adherence to medication regimens and conduct medication audits and refills.
- Document all adverse effects and interactions of medication in client records.
- Review and approve treatment recommendations made by clinical staff when necessary.
- Maintain timely and accurate documentation of assessments, clinical interventions, and discharge summaries as per medical staff regulations.
- Monitor risk indicators and take prompt action to address risk management concerns to ensure the safety of clients, staff, and visitors, safeguarding the agency from adverse events.
- Adhere to safe and evidence-based prescribing practices, obtaining informed consent for medication trials within the nurse's scope of practice.
- Act as a liaison, maintaining effective communication with external healthcare providers, primary care physicians, and family members as required.
- Monitor substance screens and perform other necessary lab work.
- Fulfill any other duties as assigned.
Minimum Requirements:
- Hold a valid Licensed Practical Nurse (LPN) license in the state of Connecticut with no current sanctions.
- Minimum of 1 year of experience in a related healthcare field.
- Knowledge and experience working with individuals facing substance use and mental health challenges is essential.
- Prior experience working with individuals in a residential setting is preferred.
- Exhibit the ability to quickly adapt to new systems and programs.
- Possess excellent verbal and written communication skills.
- Demonstrate strong customer service skills.
- Capable of developing clear and presentable documentation.
- Willingness to establish and maintain effective working relationships with others as part of a collaborative team.
