POSITION SUMMARY:
Are you a LPN who is looking to expand your skills with nursing administration in the home healthcare field?
This position will require the LPN to travel to our patients' home to complete supervisory visits with our clients receiving personal care assistance.
A company car and fuel will be provided.
JOB DESCRIPTION:
- Conduct Initial and Annual Skills Training of PCAs.
- Re-evaluation of patients skilled nursing/PSS needs
- Work autonomously with minimal assistance by the Clinical Manager
- Maintain patient confidentiality
- Make decisions based on established policies and procedures
- Preparation of the clinical progress notes.
- Informing the family and Registered Nurse of changes in the patients condition or needs.
- Supervisory Visits and teaching in Patient's Home.
- Administration of medications and treatments as prescribed by a physician in accordance with currently accepted standards of medical practice
- Total patient care
- Patient and family teaching.
- Other services in accordance with and as outlined in the Georgia Licensed Professional Nurse Practice Act.
- Reports directly to the Registered Nurse and Clinical Director.
- Licensed Practical Nurse (LPN)) must hold a current unencumbered Nursing license to practice in the state of Georgia.
- LPN will complete the assigned onboarding nursing quiz.
- LPN should have experience caring for medically fragile individuals.
- It is preferred that the LPN have experience in the home health service field.
- Current BLS Certification.
- Working knowledge of the policies and procedures of ICWP, CCSP, SOURCE, GAPP.
