Job Location: Remote / Home based. (Prefer the North Atlanta Metro-County area)
Type: Long-term 1099 Independent Contractor
Our Practice continues to rapidly grow and is looking to add another LPN to join our team. We focus exclusively on serving geriatric patients residing in memory care, assisted and independent living communities by providing primary care and managing their chronic health conditions.
Essential Job Functions:
- Detail Oriented. Must be comfortable working with EMR platforms and computers to accurately enter and update clinical data in patient charts.
- Analytical skills. Must be able to understand and follow medical charts and diagnoses.
- Interpersonal skills. Must be able to discuss patient information with other medical personnel and can utilize Medical Terminology.
- Responsive to patient care related issues and be customer service oriented. Timely communication to team members as well as those calling into the Practice is extremely important in this role.
- Telephone communication skills. You will often interact with other professional medical providers, patient’s family members or the patients themselves.
- Understanding of HIPAA, Privacy, Safety, and Compliance guidelines and exercise professional work ethics.
- Strong knowledge of chronic medical conditions is extremely helpful in this role.
- Operate office equipment such as voice mail messaging systems, email, and use of various software applications to support operational processes.
- Perform other duties or tasks, as assigned, or required.
Education / Experience:
- Graduation from an accredited nursing program (LPN) with an active GA license.
- Experience working in a Home Care or Long-Term Care type environment, home visit experience or working with seniors in other comparable settings is preferred.
