12-week assignment. Mon-Fri. No weekends. Rare but if needed -holiday/OT possible. Inpatient discharge planning hospital experience. Must have Epic and Allscripts experience. Hospice, SAR referrals, LTACHs, Acute rehabs, H.D. insurance authorizations, abuse cases, shelter admissions. Can carry a case load from 10 to 30 pts independently. Masters in SW required.
