Full Description

VitalCheck Wellness, on behalf of Photon Medical Group, is seeking a culturally sensitive and compassionate Licensed Master Social Worker (LMSW) to join our team. Patient care will be provided in-person and virtually at one of NYC Health + Hospitals' Humanitarian Emergency Response and Relief Centers (HERRCs) making a difference in the lives of migrants.

Responsibilities

• Conduct culturally competent short-term counseling, crisis intervention, risk assessment, safety planning, and psycho-education, including:
  • Crisis intervention
  • Identify need and make referrals, for outpatient/inpatient care and hospital transport
  • Trauma triage/trauma informed assessment
  • Peer support
  • Conducting emergency evaluation/assessment for:
    
  • Mental health and mental health crises
  • Substance use/disorder
  • Occupational needs
  • Domestic violence
    

• Advocacy on behalf of the family when working with NYC Administration for Children's Services or other preventative services.
• Communicating and collaborating with external service providers to enhance engagement and ensure quality of service delivery.
• Providing education and training to other staff on mental health and holistic support needs and benefits.
• Facilitating family meetings with the family and HERRC staff to discuss planning efforts and progress.
• Coordinating with emergency services, hospital Social Workers, and discharges, particularly around laboring individuals and newborns.
• Coordinating on domestic violence cases, particularly in instances where a 911 call must be made.
• Handling transfers to domestic violence shelter.

Requirements

• Master's in Social Services, Mental Health Counseling, and/or Marriage and Family Therapy.
• Licensed Clinical Social Worker (LCSW), Master of Social Work (MSW), or Licensed Master Social Worker (LMSW) certification.
• Experience in a healthcare or hospital setting is highly preferred.
• Bilingual in Spanish a Plus
• Knowledge of New York State regulations and guidelines for social work practice.
• Strong interpersonal and communication skills.
• Ability to work effectively in a fast-paced and dynamic environment.
• Empathy and sensitivity to the needs of diverse patient populations.
• Proficiency in electronic medical record systems and documentation.
• Must be able to commute to New York City and Boroughs.
