Job ResponsibilitiesCareers at DHEC: Work that makes a difference!Pursuing Excellence, Inspiring Innovation, Promoting Teamwork and Embracing ServiceUnder the general supervision of the TB Program Manager, provides social work services for persons receiving care for TB and/or HIV/AIDS. Works with the TB Nurse Case Manager to promote completion of recommended treatment, including providing Directly Observed Therapy (DOT). This position requires role-based access to SCION, CARES, and the EHR for surveillance activities, case reporting, verification of diagnosis and treatment status, and documentation of patient care activities.Provides psychosocial assessment and follow-up services to a diverse population of clients and their contacts to include substance abuse and mental health needs.
Provides short-term therapy to assist individuals/families adjust to health-related problems, assists clients in developing long range plans to address health problems, makes appropriate referrals to community resources and develops discharge plans when social work services end. Goals and treatment plans are set in collaboration with the individual/family. Documentation will meet policy standards and patient confidentiality will be maintained.
Provides patient education, initiate referrals and counsel individuals, caregivers and families in the clinic, household and community setting, and assists with contact investigations as needed.Provides Directly Observed Therapy (DOT) for treatment of tuberculosis infection and /or disease. Follows the policy /procedure for DOT, either in the clinic setting or in the community. Immediately notifies nurse if problems with treatment including missing a scheduled visit for DOT.Facilitates patient access to community resources and assists in discovering resources for person with active and suspected TB, LTBI or HIV/AIDS.
Participates in the development of the Social Work program policies as indicated.Prepares written reports and clinical documentation according to agency and program guidelines. Submits PCAS, SCEIS, and productivity on time each month.Participates in community initiatives that address public health issues, including emergency preparedness.Other duties as assigned to include emergency response activities (i.e. shelter duty)Minimum and Additional RequirementsState Minimum Requirements: A bachelor's degree in a social work or social welfare program accredited by the Council on accredited institution, plus experience as a professional Social Worker.Institutions of Higher Learning must be recognized by the Council for Higher Education Accreditation.Agency Additional Requirements: A bachelor's degree in a social work or social welfare program accredited by the Council on Social Work Education is required.
Additionally, the individual must be licensed as either an LMSW (Licensed Master Social Worker) or an LISW (Licensed Independent Social Worker) by the South Carolina Board of Social Work. Experience as a professional Social Worker is also necessary.Must have extensive knowledge of SW principles, practices and techniques; developmental needs of individuals/families, family systems theory, family-centered care, group work, community organization and cultural competence. Ability to apply knowledge to develop/implement or assess intervention plans to integrate work with other disciplines/agencies.
Must be able to work with an interdisciplinary team; relate to others in a culturally competent manner. Must have basic computer skills and be able to communicate effectively, orally and written.Must be willing to adjust hours as needed. Travel is required.
May be in contact with an infectious/potentially infectious TB cases/suspect. Must be able to pass a fit test. Must be able to use appropriate respiratory protective equipment.
Home visits required. May have to access patient homes by use of stairs. May come in to contact with domestic animals in home.
Current driver's license or ability to obtain transportation to fulfill job duties.
