GENERAL DESCRIPTION: The Outpatient Therapist at Evara Health plays a vital role in helping individuals cope with emotional, mental, and behavioral issues. Therapists provide individual, family, and group therapy services in an outpatient setting, working with both children and adults. The outpatient therapist assesses, diagnoses, and treats patients, helping them to develop coping strategies, improve their mental well-being, and lead more fulfilling lives.
ESSENTIAL DUTIES & RESPONSIBILITIES:
• Adheres to current, applicable evidence-based clinical guidelines, standards of care, and standards of practice
• Performs biopsychosocial assessments; utilizes standardized screenings and assessments
• Develop and implement individualized treatment plans tailored to each patient's unique needs and goals. Continuously evaluate clients' progress and adjust treatment plans as necessary.
•Provides crisis intervention, risk assessments, safety planning, and Bakers Acts as needed
•Provides individual, couples, and family therapy to clients of all ages, addressing a wide range of mental health and substance concerns. Services are culturally competent, strength-based, and Solution Focused.
• Participate in treatment team meetings, supervision sessions, and provider meetions to enhance clinical skills and knowledge.
• Remains current in the general knowledge of psychotropic medications, side-effects and DSM diagnostic criteria.
• Coordinates efforts between multiple service providers when appropriate and consented to.
• Maintain accurate and confidential client records, including progress notes, treatment plans, and other required documentation.
• Work closely with other healthcare professionals, such as psychiatrists, Social Workers, and primary care providers, to ensure comprehensive care.
• Adhere to ethical guidelines, including maintaining confidentiality and informed consent, and ensuring the well-being of clients.
• Stay updated with the latest research, treatment modalities, and therapeutic techniques through ongoing training and professional development.
• Provides in-service presentations to team members as needed
• Actively participate in assigned Evara Health and community meetings and committees
• Prepare and present patient case reviews as requested
• Acts as a resource for Evara Health employees and community partners
• Responsible for providing top-notch customer service in line with Evara Health policies and procedures, standards, and expectations.
• Responsible for accurate and timely documentation within patient records as outlined in workflow and policies and procedures
• Openly, clearly, and respectfully share and receive information, opinions and concerns and feedback in a supportive manner.
• Work collaboratively by actively participating in huddles, team meetings, by mentoring new and existing co-workers, building bridges, and creating rapport with team members across the organization.
• Effective and timely handling of other duties as assigned.
• Maintains strict confidentiality as appropriate.
• Strict adherence to all Evara Health, HIPAA, HRSA, State, Federal and accreditation agency rules, and regulations.
MINIMUM QUALIFICATIONS:
• Minimum of 1 year of experience
• Preferred: Experience in a healthcare setting
EDUCATION AND EXPERIENCE:
• Master’s Degree accredited by the Accreditation of Counseling and Related Educational Programs (e.g., social work, clinical mental health counseling)
LICENSES, CERTIFICATION OR REGISTRATION:
• Current, unrestricted Florida Licensed Clinical Social Worker (LCSW)
• Current BLS certification required.
