Lcsw - Licensed Clinical Social Worker

Integrated Health Care Licensed Clinical Social Worker, LCSW, for our Primary Care Center in Broward County *Competitive Compensation and Benefits Package
-Works collaboratively with the primary care and behavioral health team to ensure that ongoing liaison occurs with clients, medical providers, and families.
-Completes documentation (including but not limited to Master Treatment Plan, Treatment Plan Reviews, ASAMs, MHOMF, CFARS/FARS, Discharge Summaries, as applicable) for assigned cases in accordance with time frames established by licensure, regulations, contracts, Agency policies and procedures, and/or third party payers.
-Completes and submits timely prior authorization requests and concurrent authorization requests to the Utilization Management Department to ensure services provided are authorized and reimbursable.
-Documents all services in the Electronic Health Record within the defined timeframes.
-Completes documentation to demonstrate medical necessity and fidelity to the Agency’s adopted Evidenced Based Practices (EBP’s).
-Participates in clinical supervision and team staff meetings as scheduled.
