Summary/Objective: 

The Extensivist Care Program creates an environment for regular monitoring and interventions for the various conditions the highest-risk Medicare Advantage patient may have. The Licensed Clinical Social Worker (LCSW) will identify and address behavioral, substance abuse, and social determinants to improve health outcomes. 

 

Essential Functions:

  • Maintain a specific caseload of supported patients, including initial engagement and ongoing longitudinal assistance and case management. 
  • Complete required assessments and documentation within specified timeframes.
  • Primarily engage Patients in the patient's home setting and virtually to identify and address needs related to behavioral, substance abuse, and/or social determinants of health. This may include some brief interventional therapy as appropriate and within the scope of licensure.
  • When needed, provide direct social work services and support to Patients by conducting home and residence visits as assigned, including Home Safety and Wellness Assessments, Care Planning, and on-going re-assessments of Patient care to support immediate and long-term care needs
  • LCSW partners together with the Patient's assigned APRN and Extensivist Physician to refer to appropriate no- or low-cost community services that support health, independence, and resolution of care barriers,  such as Meals On Wheels (MOW), local Area Agencies on Aging (AAA), Senior & Assistance Programs, and transportation resources
  • Involves the Patients' families,  caregiver(s), and support systems as needed to achieve the health outcomes
  • Effectively collaborates with all those involved in the Patient's care, including health services contractors (i.e., Home Health, Hospice, and Community Agencies), to meet the Patient's care goals and further success
  • Consults with and advises the Extensivist Team regarding the relationship of social, emotional, and cultural factors to health and medical care, and to the availability of social services in the community
  • Assists APRN and Extensivist Physician in supporting the Patient Plan of Care, including education and completion of Advance Directives (i.e., Living Will, Healthcare Surrogate, DNR, etc.) as appropriate
  • Maintains current progress notes and other documentation in the Patient's Electronic Medical Record (EMR)
  • Coordinates referrals to behavioral and substance abuse Specialists when appropriate
  • Create an individualized plan of care for each patient to resolve barriers to care, build additional educational or contingency planning and engage with social support systems as appropriate. 
  • Meet required Key Performance Indicators and other departmental goals, including phone audits. 

 

Supervisory Responsibility:

  • No direct reports
  • LCSW to report to the VP of Health Services (Operational) and the Extensivist Physician (Clinical)

 

Physical Demands:

  • Manage and coordinate inbound and outbound telephonic outreach
  • Ability to document in Electronic Database systems.

 

Position Type/ Expected Hours of Work:

  • This is a full time position and core hours of work and days are Monday to Friday 8:00 a.m. - 5:00 p.m.

 

Travel:

  • Approximately 10% of travel required to the Patient's residences on occasion, which may include nursing homes, assisted living facilities, and Alternate Living Facilities (ALFs)
  • Travel to Corporate or Regional Offices may be required for training or meetings.

 

Required Education/ Experience: 

  • Master's Degree in Social Work
  • Must have a valid Florida LCSW license with no restrictions
  • Two years of social work experience in health care, and a minimum of one-year experience working with a geriatric population

 

Additional Eligibility and Qualifications:

  • Age-specific competency in working with the elderly and knowledge of community services for the elderly and their families
  • Bilingual in Spanish/English preferred but not required
  • Knowledge of social work principles and practices, including case management and counseling techniques
  • Ability to use Electronic Medical Record (EMR) system to document Patient encounters
  • Effective communication and interpersonal skills
  • Valid Florida Driver's License and auto insurance
  • Satisfactory completion of a pre-placement Tuberculosis test is a requirement for this position
