Requirements:
- Bachelor's Degree in Social Work
- Active KS license
- Valid Drivers License
- One year experience with psychiatric patients preferred
Responsibilities:
- Meet with patients and the interdisciplinary team to provide updates to family and community partners.
- Gather and document information in the social service assessment and discharge plan.
- Create safe and healthy discharge plans.
- Coordinate specialized aftercare.
- Serve as point of contact for patients’ wellbeing with members not on the care team.
- Act as the key informational flow regulator regarding a patient’s progress in treatment.
- Perform care consistent with the scope of licensure and in accordance with the facility's policies and procedures.
- Work effectively with fellow medical staff and treatment team members.
- Any other duties as assigned.
