Summary:
This service consists of community-based Mental Health and substance use services, and structured rehabilitative interventions intended to increase and restore an individual’s ability to live successfully in the community. The team approach involves structured, face-to-face therapeutic interventions that assist in reestablishing the individual’s community roles related to the following life domains: emotional, behavioral, social, safety, housing, medical and health, educational, vocational, and legal.
Conduct comprehensive substance use assessments considering the relationship between substance use and Mental Health
- Use cognitive behavioral approaches and relapse prevention to work with individuals in later stages of change readiness
- Show knowledge and use of clinical process to achieve positive outcomes to include: evaluation, assessment, service, treatment planning, intervention techniques, aftercare, and transition planning
- Remain available face to face or by telephone, 24 hours a day 7 days a week as staff serves as the first responder in the event of crisis situations. The service site will be at any location the consumer or family needs
Education and Experience Required:
- One (1) year of professional experience with the adult Mental Health population
Licensure/Certification Requirements:
- LCAS, LCASA, CADC
