- Demonstrates culturally competent and developmentally appropriate patient care.
- Practices collaborative relationship with the patient, family, interdisciplinary care team and other service providers.
- Acts as a consult and liaison for complex psychosocial cases and assists in resolving barriers to care involving patients who are seriously ill and have challenging medical and social needs.
- Completes a biopsychosocial-spiritual assessment and develops a patient-centered care plan.
- Communicates significant psychosocial findings, recommendations and interventions with the interdisciplinary care team.
- Intervenes in crises, mediates and resolves conflicts between patient, family/caregiver and interdisciplinary team.
- Organizes and facilitates family meetings, as indicated.
- Assists in clarifying communication, values, goals of care and healthcare decisions.
- Documents all patient/family-related encounters, interventions and outcomes in patient's electronic medical record.
- Offers brief psychotherapeutic counseling for individuals, couples and families and as needed, offers support to the care team during challenging cases.
- Educates patient and family about palliative and hospice care philosophies and provides psychoeducation and nonpharmacological strategies for coping with serious illness.Assists in discharge planning, coordinating care and helping patient/family navigate systems.
- Facilitates advance care planning and lifespan planning.
- Participates in interdisciplinary team meetings including care planning and ethics consultation.
- Facilitates patient linkages to relevant resources within the hospital and throughout the continuum of care.
