Position Summary:

This Intensive Case Management position is assigned exclusively to Mental Health Court, a specialty court in the 6th Judicial Circuit. The Intensive Case Manager (ICM) will be the primary contact for all referrals from Mental Health Court to DFL, the ICM is a part of a teaming model to include an identified Counselor. The ICM will provide all services necessary to properly coordinate and monitor service delivery to individuals referred to and accepted into Mental Health Court. The ICM will be responsible for collaborative coordination with all members of the Mental Health Court team, to include the dedicated DFL Counselor, and all staff of the Mental Health Court. The ICM is responsible for remaining up to date on all action of the court, to attend and participate, as needed, in all court proceedings on behalf of and in support of the individuals served by Mental Health Court. The ICM will meet with individuals served by Mental Health Court in the community at a minimum of weekly for the first 45 days, including visiting individuals in jail, treatment programs, or in their home. In that time, the ICM is responsible to identify the needs, in particular the unmet needs, of the individuals served in Mental Health Court and to take all the steps necessary to connect the individuals with the resources needed to address their needs. In particular, the ICM will work to connect individuals with a health plan if needed to ensure that their physical and Mental Health needs are address, and all available entitlement benefits such as food stamps, social security or social security disability insurance, etc. The ICM will work in partnership with the entirety of the Mental Health Court team to identify, and secure permanent stable housing as needed. Once the individual is connected to the appropriate level of services in the community, all direct service care will be transferred. However, the ICM will be responsible for seeking and receiving quality updates from all service providers on how the individual is doing and to speak with the individual a minimum of every 2 weeks for an update on how they are doing. The ICM is responsible for addressing any barriers or difficulty the individuals may be experiencing with their identified service providers, and for informing the court and the full Mental Health Court Team of all progress and or regress. The ICM is responsible for linking, coordinating, monitoring, educating and empowering all individuals served by Mental Health Court.

EDUCATION EXPERIENCE/REQUIREMENTS:

  1. Bachelor's degree in Human Services from an accredited college with a minimum of one year of previous experience working with adults with serious persistent mental illness.
  2. Completion of Behavioral Health Case Management Certification by the Florida Certification Board (within 1 year of hiring).
  3. Must have excellent writing, communication, computer, organizational and time management skills with the ability to work independently and as part of a team.
  4. Proficiency in completing progress notes, treatment plans, and pertinent information concerning the client required.
