WHAT YOU CAN EXPECT
Describes the PACE program, either by telephone, electronically, or in-person to interested persons including candidates for enrollment, caregivers, or others in the community interested in the PACE program.
Escalates unresolved issues to supervisor and/or manager. Collaborates with other teams to resolve issues.
Serves as a liaison between the hospitals, physicians, different clinics, as well as community agencies to ensure continuity and quality of program services.
Answers initial calls for PACE referrals and inquiries.
Referral tracking and follow up.
Collaborates with Intake Coordinator to schedule initial in person assessments.
Verify Medicare benefits.
Work with applicants on initial Medicaid application.
Work with participants on annual Medicaid redetermination.
Follow up with IDT regarding expired authorizations.
Assist with Enrollments into system as back up.
Notify IDT of schedule updates or changes immediately as they occur.
QUALIFICATIONS
High School Diploma/GED- Required
Associate's Degree- Preferred
1 year Experience in a PACE program- Preferred
1 year Experience with Medicaid- Preferred
1 year Experience in a health-related setting- Preferred
