DESCRIPTION:


The Community Resource Center is a unique department at SAC Health that aims to address health inequities within the community such as lack of access to care, food insecurity, homelessness and housing challenges, among many other issues, in the San Bernardino region. All staff of the department is required to have basic knowledge of the Social Determinants of health, and its impact on underserved communities. The primary function of this position is to serve as a triage and assist with clients who come into the Community Resource Center by connecting individuals and families in need to internal and external resources to meet those needs. Duties will include the completion of the CRC intake process, including a thorough client assessment, developing of service plans, monitoring of progress and completion of plan, and referring to other department specific programs, where necessary. This position includes participation in community-based activities and health fairs, participating in CRC programs and events, and working closely with other members of the CRC and SAC Health staff. Tasks include maintaining accurate documentation, researching and maintaining a resource directory. Performs other duties as assigned.

Schedule: 4 days per week, 10 hour days


ESSENTIAL FUNCTIONS AND DELIVERABLES:


  • Conducts intakes and initial interviews with patients and families to perform psychosocial assessments in a culturally sensitive manner to ensure that fair, equal care and accurate assessments are rendered.
  • Thoroughly and accurately assesses problem situations. Uses purposeful strategy to intervene in problem situations. Communicates clearly and concisely both verbally and in writing. Evaluates basic needs required to stabilize family unit and develops timely plans. Provides referrals and linkages. Answers inquiries about SACHS and CRC.
  • Provides culturally sensitive education to ensure fair and equal care to all clients in the development of appropriate service plans and referral and linkages to internal and external resources based on individual/family needs.
  • Provides families with necessary case management information, facilitates referrals to community resources, and serves as liaison among clients, integrative health care staff and community agencies.
  • Documents encounters with clients in their respective charts. Provides accurate and timely documentation according to departmental guidelines. Maintains accurate and timely records of client activity.
  • Understands the Social Determinants of Health (SDoH) when seeking to address the needs of clients.
  • Attends and participates in daily/weekly CRC staff meetings, monthly CRC Department staff meetings and SACHS General Staff Meetings, in-services, staff development and participates in educational courses, workshops and conferences.
  • Attends and participates in community-based meetings as assigned. Promotes services and educational opportunities at area health/resource fairs and events.
  • Demonstrates professional development on the job as evidenced by staying current with the field through journals, articles and pertinent literature. Researches and maintains active directory of community resource and referral agencies.
  • Use of EPM and EHR as appropriate.
  • Other duties as assigned.


QUALIFICATIONS:


  • Education: Associate Art Degree in Social Work, Human Services, Social Services, Public Health, or a related field is required.
  • Licensure/Certification: Requires a valid current California driver's license, current motor vehicle record and proof of auto insurance. Must maintain a good driving record and current auto insurance. As a requirement of this position you must receive EPIC certification for the module you have been hired into.
  • Experience: 2 years of related work experience is required. 2 years of clinical and or case management experience preferred. Bilingual skills in English/Spanish Preferred.
  • Essential Technical/Motor Skills: The ability to use a computer on a routine basis and complete required documentation. Experienced with Microsoft Outlook, Excel, PowerPoint, and Publisher. Strong communication skills, both orally and in writing.
  • Interpersonal Skills: Strong interpersonal relations skills. Fluency in English and Spanish preferred. Ability to maintain a professional attitude in difficult situations and communicate effectively. Understands needs of clinic and works well in a team environment.


Must be legally authorized to work in the United States on a full-time basis. Must not now or in the future require sponsorship for employment visas.
