POSITION SUMMARY: The Intake RN is responsible for the accurate and timely assessments of potential participants. This position functions as a member of the Intake/Enrollment team and is vital to ensuring the smoothest possible transition for new participants into the program as well as informing the team on the needs and/or safety concerns present at time of admission.

RESPONSIBILITIES AND DUTIES:
ESSENTIAL JOB FUNCTIONS:
- Serves as the initial clinical point of contact with potential participants and representatives (if applicable) to create an initial assessment and/or safety screening
- Conducts referral assessments in the home and documents observations in the assessment such as condition of home, both inside and outside
- Works directly with the Interdisciplinary Team (IDT), participant, and his or her representative to communicate goals and present an accurate initial picture that leads to an appropriate plan of care
- Communicates to the family or care giver and provides appropriate patient education on services and types of care provided
- Provides patient education related to medications, inhalers/nebulizer treatments, and point of care testing such as blood sugar and INR
- Observes, records, and reports participants condition and reaction to drugs and treatments to medical providers, using SBAR format
- Documents assessments, care, and care coordination in the EHR
- Completes regulatory assessments of participants (initial, annual, and semi-annual), as well as hospital, ED, and fall follow up assessments
- Attends and participates in Interdisciplinary (IDT) team meetings as assigned

Qualifications
QUALIFICATIONS AND REQUIREMENTS:
- RN license in good standing with the state of Colorado Required
- 1 year minimum experience utilizing nursing skills and/or performing assessments
- Reliable transportation required
- CRRN and/or Rehab background highly preferred
- Experience working on an intake team highly desired
- Knowledge of Long Term Medicaid services desirable
