Hiring for multiple locations - Madison Heights and Dearborn Heights

Typical schedule will be Monday thru Friday 8:30am to 5:00pm and/or 11:00am to 7:00pm

Seeking candidates with Bachelors Degree (minimum in a relevant field), Masters Degree preferred, CADC or MCBAP development plan required. Must complete MAFE certification.

All services will integrate substance use disorder, mental health and primary health care treatment in cohesive and client-centered programs.

JOB TITLE: Intake Coordinator

Position Summary

Under general supervision of the program director and director and direct supervision of the Site Director, the Intake Coordinator is responsible for assessing, scheduling, and orientation of admissions. Participate as a member of the multi-disciplinary team providing client assessments and counseling.

Key Tasks and Responsibilities

• Use the LIGHT Model in all counseling interactions.
• Provide Site Director with a copy of PNLH’s updated waiting list daily.
• Provide Site Director with a list of anticipated weekly admissions by Friday afternoon (number, days, any special issues regarding these clients).
• Provide all clients on waiting list group with a list of interim services (i.e. HIV Ed., etc.).
• Inform all clients on waiting list of waiting list group.
• Complete all admission papers within the same day.
• Return all phone calls within 24 hours. Under rare circumstances where you have to numerous phone calls to return, inform Site Director.
• Client applicants who do not pass urine screens need to be referred for assistance (Make sure they have emergency phone number lists, food, shelter, AA/NA meetings, Etc.).
• All clients referred out for services need to be documented in Referral Log.
• Client ineligibility reports need to be documented in Referral Log.
• Have all admission confirmations to probation officers, EAP’s, referrals sources, etc. out within 48 hours.
• All client files and confidential information needs to be locked in appropriate file.
• All clients information needs to be neatly, correctly and completely input in the clients’ database.
• Participate in staff meetings.
• Communicate with clients families, significant others, and other Personalized Nursing LIGHT House personnel in order to gather pertinent information, explain client care plans, and coordinate activities.
• Participate in clients care audits or reviews as assigned.
• Perform other duties as assigned.

Educational Requirements

Bachelors Degree required, Masters Degree preferred, CADC or Mcbap development plan required. Must complete MAFE certification.

Experience/Requirements

Good verbal and communication skills. Minimum of six-months experience with substance abuse users.

PNLH expects that the majority of our clients will have co-occurring mental health and substance abuse disorders. Therefore each clinician will seek training to be competent with this population.

Job Type: Full-time

Salary: $18.00 - $22.00 per hour
