Hospice Patient Services Coordinator
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
- Maintains patient charts.
- Maintains charts in organized form through current filing and management of all appropriate forms.
- Performs office functions.
- Accurately performs secretarial duties (i.e. correspondence, typing and adding monthly mileage).
- Maintains inventory of office supplies & medical supplies.
- Verifies current license of physicians with temporary privileges to appropriate institutions and/or licensing agencies.
- Call or fax physician orders or request.
- Send referrals to appropriate disciplines.
- Patient chart registration.
- Insurance verification/precept.
- Aide scheduling.
- Send and track physician orders.
- Answer telephone and give factual information pertaining to Home Health according to the nature of the questions and within guidelines of regulations.
- Orders equipment for patients.
- Prepares Medical Records for storage.
FUNCTIONS AS CO-OPERATIVE MEMBER OF HEALTH CARE TEAM
- Understands and maintains client confidentiality.
- Understands and maintains open lines of communication
- Is economical in the utilization of supplies and equipment.
- Completes assignments in a timely manner.
- Cooperates with other staff members, hospital personnel and clients in a professional manner.
CUSTOMER SERVICE:
- Promotes good working relationships among staff & other department: treats everyone with respect & courtesy.
- Demonstrates Service Plus skills.
- Respects patient/employee confidentiality.
- HCHB experience a plus.

EDUCATION and/or EXPERIENCE: High school graduate. Possess typing skills and knowledge of medical terminology. Good communicative skills. Must demonstrate maturity. Computer training or experience in basic computer skills.
LANGUAGE SKILLS:
Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to effectively present information in one-on-one and small group situations to customers, clients, and other employees of the organization. Ability to read and understand the written treatment care plan, write and carry out instructions, and communicate with clients and/or families and emergency personnel. Demonstrates interest in and empathy with the home care client and family members. Demonstrates positive interpersonal skills necessary to work productively with clients, families, and co-workers.

Pay: $22.00 - $26.00 per hour

- Hospice or Home Health: 1 year (Preferred)
- HCHB: 1 year (Preferred)

- Flint, MI 48532: Relocate before starting work (Required)
