Intake Coordinator

Envision Healthcare at Home is seeking a full-time Intake Coordinator for our busy and growing home health & hospice office in the Salt Lake City, UT area.

Responsibilities include:

• Managing all aspects of the patient intake process including communicating directly with patients and families
• Data entry
• Customer service, establishing and maintaining positive relationships with customers and referral sources
• Responding to customer requests and concerns
• Managing the insurance verification and authorization process
• Ensures compliance with all state and federal referral /intake regulatory requirements
• Utilizes electronic medical records to access and input patient information
• Shares essential patient information with all members to coordinate care
• Obtaining authorizations for service requests from non-Medicare pay sources
• Coordinates with Marketing and Clinical Director to ensure required documentation is obtained from physician
• Participate in orientation of new employees

Job Requirements:

• High school degree or GED
• Home Health or Hospice experience highly preferred specifically in referrals/intake
• Strong computer skills with competence in electronic intake and phone triage
• Effective communication, positive attitude and team player
• Highly organized ability to multi-task
• Excellent customer service skills and commitment to quality patient care
• Knowledge of medical terminology
