Job Description
Envision Healthcare at Home is seeking a full-time Intake Coordinator for our busy and growing home health & hospice office in the Salt Lake City, UT area.
Responsibilities include:
- Managing all aspects of the patient intake process including communicating directly with patients and families
- Data entry
- Customer service, establishing and maintaining positive relationships with customers and referral sources
- Responding to customer requests and concerns
- Managing the insurance verification and authorization process
- Ensures compliance with all state and federal referral /intake regulatory requirements
- Utilizes electronic medical records to access and input patient information
- Shares essential patient information with all members to coordinate care
- Obtaining authorizations for service requests from non-Medicare pay sources
- Coordinates with Marketing and Clinical Director to ensure required documentation is obtained from physician
- Participate in orientation of new employees
Job Requirements:
- High school degree or GED
- Home Health or Hospice experience highly preferred specifically in referrals/intake
- Strong computer skills with competence in electronic intake and phone triage
- Effective communication, positive attitude and team player
- Highly organized ability to multi-task
- Excellent customer service skills and commitment to quality patient care
- Knowledge of medical terminology
