Summary: All referrals to the Home Care department and for the accuracy and completeness of each referral. Working in collaboration with the intake team, the Intake Coordinator assesses all clinical documentation for accuracy and completeness to meet JCAHO and Medicare standards. Obtain managed care and authorizations when appropriate.
Education: In addition to the skills normally obtained through completion of a High School education, must be a graduate of an accredited school of nursing, physical therapy or occupational therapy assistant program.
Licensure: Current/Active licensure as an LPN, PTA or COTA in the state of Rhode Island. Massachusetts LPN, PTA and COTA license preferred. Active Driver’s license.
Experience: One year as a home health clinician in a certified agency with experience and knowledge of JCAHO/Medicare documentation.
Skills and Abilities: Excellent communication skills and ability to multitask. Computer skills and knowledge of Microsoft and Excel preferred.
