Job Summary/Major Function: Under the supervision of the Director of Home and Community Services, the Independent Living Skills Trainer is responsible to assist waiver participants to
become more independent in performing Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL). ILST services are individually designed to improve the ability
of the participant to live as independently as possible in the community. ILST service may include training, supervision or assistance to, an individual with issues related to self-care,
medication management, task completion, communication skills, interpersonal skills, socialization, mobility, community transportation skills, problem solving skills, money
management, pre-vocational skills and skills to maintain a household.
Responsibilities
- Conduct a comprehensive functional assessment of the waiver participant, identifying the
participant's strengths and weaknesses in performing Activities of Daily Living (ADL)
and Instrumental Activities of Daily Living (IADL) related to his/her established goals.
The ILST will be responsible for completing functional assessments at least annually with
the participants that they serve. - Implement Detailed Goal Plans derived from the functional assessment by meeting with
each participant as indicated in their service plan and monitoring progress on goals. - Complete session notes for all encounters with the participants.
- Complete collateral progress notes as needed for contact with Service Coordinator and
other service providers. - Timely submission of session notes, which document date, time, and duration of visit as
well as summarize essential information from each visit. - Complete Individual Service Reports (ISR) as required to document goals, progress, and
recommendations. - Attend and participate in scheduled Team Meetings for participants served to review the
current Service Plan twice annually as scheduled by the Service Coordinator. The ILST
is expected to notify their supervisor in the event they are unable to attend a scheduled
meeting so that coverage by the supervisor can be arranged. - Provide training as needed to all individuals involved in supporting the participant to
continue independence on achieved goals (i.e. caregivers, in-home workers, waiver
providers, etc.). - Interface with the Service Coordinator and all other service providers to maintain
communication regarding the progress of each participant served. - Meet with supervisor at least bi-weekly for regular supervision and feedback.
- Mediate emergency situations pertaining to the participant according to established
policies and procedures, and ensure communication of such situations in a timely fashion. - Ensure that each participant is treated with dignity and respect in accordance with New
York State regulations and Agency policy. - Report possible violations of participants' rights promptly, and complete and submit
incident reports in accordance with New York State regulations and Agency policy. - Assist with any investigation into incidents occurring within the program as requested,
and ensure that confidentiality is maintained in regard to such investigations. - Maintain the confidentiality of any information relating to the program participants and
staff of the program. - Ensure that the participant's needs are met and intercede on their behalf as necessary.
- Attend and complete training in accordance with New York State regulations and Agency
policy. - Perform additional responsibilities as requested and/or necessary, including staffing
alternate locations when assigned.
