```Overview:```
The Hybrid Care Coordinator role is a critical member of the CAIT Interdisciplinary Team for Remote Patient Monitoring. This role reports to reports to the Care Coordinator Supervisor, or Senior Operations Manager. Responsibilities are to ensure a positive patient experience within CAIT care programs, as well as to build good relationships with local provider groups to ensure that CAIT is a reliable and trusted partner in patient care. Responsibilities also include ensuring key clinical indicators are collected and communicated across the care ecosystem, red flags are escalated and addressed, billing thresholds are achieved and documented, and overall care plans are documented, followed, and updated as the need arises
Our company is located in Fort Worth, Texas.
Employees will be required to work some days onsite and some days from home. This position is full-time, Monday - Friday. Employees are required to work our normal business hours of 8:00 AM - 5:00 PM CST. It may be necessary, given the business need, to work occasional overtime.
We offer up to 2 weeks of paid virtual, peer - related, on-the-job training. After training is completed, you will be required to work a hybrid work schedule between home and on - site.
```Responsibilities:```
- Schedule and coordinate patient appointments, ensuring efficient use of provider's time, in both office and home settings
- Maintain accurate and up-to-date patient records in electronic medical record systems including conditions, history and medications
- Assist patients with completing necessary forms and paperwork
- Coordinate referrals and contact with other provider groups, including specialists, and follow up on patient care plans
- Communicate with patients, providers, and other healthcare professionals to ensure continuity of care
- Handle escalated patient issues, ensuring the appropriate resources are involved
- Support CAIT providers and Nurse Care Managers with operational and clinical tasks
- Manage incoming calls and inquiries, providing excellent customer service
- Assist with insurance verification and billing processes and documentation
- Inventory Management including shipping and receiving
```Experience:```
- Previous experience working as a Medical Assistant or CNA, or in a medical office clerical or medical office administrative setting preferred
- Familiarity with medical administrative support tasks such as scheduling appointments and maintaining patient records
- Knowledge of medical terminology and ability to accurately transcribe medical information
- Proficient in using electronic medical record systems, Microsoft office software applications, and Calendar applications
- Excellent communication skills, both written and verbal
- Strong attention to detail and ability to multitask in a fast-paced environment
- Ability to type at a speed of 40+ WPM (Words per minute)
- Experience with using videocall platforms, incluing Microsoft Teams
