HUMAN SERVICES PROGRAM TRANSITION SPECIALIST: YOUR ROLE

As a dedicated Human Services Program Transition Specialist with our Human Services nonprofit, your main goal is to support individuals on their journey to successful re-entry into the community. You conduct thorough assessments and use motivational interviewing to develop personalized transition plans, serving as vital roadmaps for their reintegration. In times of crisis, you step in immediately, offering support and ensuring they receive the necessary care. Collaborating with stakeholders, including the Department of Correction and community partners, you establish strong connections and coordinate services to provide participants with the resources they need. Engaging with detainees through regular presentations, you inform them about the program and available resources, giving them a glimpse of the opportunities that await upon release. Your consistent communication, whether through face-to-face meetings or phone conversations, keeps participants engaged and accountable. By documenting progress, completing transition plans, and reporting outcomes, you effectively track achievements and program effectiveness.

Working closely with Reentry Mentors and the Reentry Coordinator, you ensure a smooth transition and ongoing care for participants. Attending meetings and engaging in professional development keeps you at the forefront of best practices. As a Transition Specialist, you enter case notes meticulously, maintaining organized participant files and facilitating efficient reporting. Your dedication to empowering individuals and making a positive impact makes the role of a Transition Specialist immensely fulfilling. Through personalized support, service coordination, and open communication, you contribute to the successful reintegration of individuals into the community, fostering a brighter future for all through our Human Services nonprofit.


WHAT WE NEED FROM YOU

We are looking for a Human Services Program Transition Specialist who exhibits compassion and a love for helping people. We need someone with great communication skills and leadership skills. We are also looking for the following requirements:

  • Bachelor's degree in business, management, or related field from an accredited college or university
  • 3+ years of demonstrated ability in managing and operating an effective adult learning, workforce development, or employment-placement environment
  • Strong writing skills
