Job Description:
OVERALL OBJECTIVE OF JOB:
Case Aide for the Protective Services unit and Care Management/OPTIONS program.
ESSENTIAL FUNCTIONS OF JOB:
- Shared Case Aide for Protective Services and OPTIONS programs. Assignment in either program may shift depending on programmatic needs at any given time.
- Complete SAMS training.
- Protective Services
- Complete Protective Services training and maintain certification.
- Taking Reports of Need, categorizing, and sending to the PS
Supervisor immediately.
- Help with Protective Services Case Note entry into the SAMS database.
- Organize paper files.
- Assist with purging process of PS files.
- OPTIONS
- Small case load of meals-only OPTIONS consumers who require a NAT-E assessment.
- Manage waitlist for OPTIONS and send to providers as needed and record in consumer files.
- Assist with OPTIONS Care Managers, including scanning into the appropriate file and case note entry.
OTHER DUTIES OF JOB:
- Uses computer and other office equipment.
- Performs other job-related functions as required.
- May be assigned to assist in other areas of the Agency.
QUALIFICATIONS:
Minimum Experience and Training:
One year as an Aging Case Aide 1;
or
Two years of public contact work, including one year in a human service program;
or
Any equivalent combination of experience and training.
