Social worker will:
- Provide individual counseling, group meetings and sessions to adults who struggle with co-occurring conditions of mental health and substance use.
- Support individuals with a history of severe mental health conditions, who are often among other marginalized populations (chronically homeless, HIV+, former foster youth, etc.)
- Support individuals in maintaining housing stability, improving quality of life and reduce the need for psychiatric hospitalization.
- Coordinate and link to resources such as specialty mental health and substance use disorder treatment.
- Provide crisis and case management.
- Complete clinical documentation.

About the Supported Housing Team
The Supported Housing Team provides support to over 150 tenants in permanent housing dedicated for individuals with low-income status and psychiatric disabilities – including- chronically homeless, former-foster youth, and people with chronic medical issues. The team facilitates house meetings and assists tenants in maintaining stability in their housing and in connecting to the community. We are a small (4-person) team which prioritizes meeting clients where they are at.
