Housing Specialist

Title: Housing Specialist Location: West End

Starting Annual Salary: $54,700 - $55,700.00

Sandhills Center is a Local Management Entity-Managed Care Organization (LME-MCO) serving people in nine counties in central North Carolina. We have a robust network of providers who offer treatment and support for mental health, intellectual/developmental disabilities and substance use disorders. It is the goal of Sandhills Center to make treatment options available to those who need them, to enrich and support our Provider Network, and to contribute to the overall well-being of the communities we serve.

Duties:

· This position is responsible for insuring prompt and seamless care coordination for housing of the Transition to Community Living members served.

· Will be responsible for providing assistance to providers and members with re-housing issues, participating in transition meetings, engaging in recovery/service planning, and sharing of housing resources.

· This position will also carry a caseload of people who have made transition to the community from institutional settings. Requires travel in catchment area.

Minimum Requirements:

· Qualifications are Master's degree in psychology, social work, education, health, or related human service field, and two years of experience in professional level treatment programming;

· or Bachelor's degree in one of the fields listed and three years' experience as indicated above
