Our Hospice team has an immediate opening for a an Intake Coordinator to support our intake and admissions teams.
Job Responsibilities include but are not limited to:
- Taking referrals via phone, fax or email and entering them into the EMR, auditing admissions for compliance, obtaining certifications and past medical history from physicians
- Evaluating appropriateness of all referrals for information visit/admission assessments; referring to other community resources and JSSA as appropriate
- Triaging needs of hospice admission nurses while in the field
- Obtaining Verbal Certifications from Physicians for start of care of hospice
- Acting as an ambassador for the Hospice Program and our organization when communicating with patients, families, and physicians.
- Able to work a flexible schedule of four 10 hours days including every other weekend.
- Minimum of 2 years of intake experience (within hospice, homecare of similar healthcare environments preferred)
- Strong written and oral communications skill
- Excellent computer skills – Microsoft Office 365, EMRs, databases, and more
- Excellent phone customer service skills
- Strong organizational and multitasking skills
- Ability to work in a fast-paced environment with an emphasis on collaboration and maintaining positive relationships with colleagues and clients
- Ability to actively listen and concisely explain complex information to clients
