Family Case Management Coordinator, (Parents as Teachers)

Position Summary: The Family Case Management Coordinator is accountable directly to the Home Visiting Program Manager. Provides home visitation services and ensures Family Case Manager's activities meet the requirements of Parents as Teachers. Utilizes Parents as Teachers (PAT) approach to promote positive parent-child interactions and healthy childhood growth and development. This is a full-time position.
 
Salary – Indeed provides its own estimated salary calculator and is not affiliated with COH’s range. The salary for this position starts at $57k and up, based on experience.
 
Highlighted Duties and Responsibilities:
  • Establishes a caseload of at least twenty families beginning no later than five months after onboarding has been completed.
  • Participates in outreach to parents and families both within COH programs and in the community to ensure caseload size and program enrollment is maintained. 
  • Develops PAT Family Synthesis Record based on areas of need identified during the family’s first 90 days in the program in collaboration with the Home Visiting program manager for all families assigned to caseload.
  • Develops Family Goal Plans with assigned families, monitoring progress and barriers to success. 
  • Networks with community resources for housing, employment, substance abuse treatment, mental health treatment, training, child and family services, etc., and makes referrals as appropriate. Follows up on referrals documenting progress in the DCRS. 
  • Promotes positive parent-child interactions and introduces activities to build parenting confidence through regular home visits utilizing the Parents as Teachers model and curriculum. 
  • Completes required assessments and screenings with families at designated intervals, i.e., PHQ-9 Depression Scale, Abusive Behavior Inventory (ABI), Ages and Stages Questionnaire, KIPS etc. and enters scores for assessments into DCRS database. 
  • Enters data each week into DCRS data base and provides information at least monthly for funder reports DOH reports, and case reviews. 
  • Attends clinical, departmental, program and care team meetings and other organizational meetings and trainings as appropriate. 
  • Works collaboratively with prenatal services team to ensure patients receive Home Visiting services in a timely manner. 
  • Makes referrals for health services to Community of Hope health services as needed. 
  • Provides support to Family Case Managers to ensure that the required number of home visits are conducted on a monthly basis and all documentation is completed accurately and timely.  
  • Supports the Parents as Teachers Quality Endorsement and Improvement Process (QEIP) under the guidance of the Home Visting Program Manager by contributing to the development of policies, data collection, data analysis, and ensuring that Family Case Manager’s activities are conducted with fidelity to the model.  
  • Conducts initial shadowing of home visits with newly hired Family Case Managers during onboarding and shadows visits annually for each home visitor thereafter. 
  • Promotes the “One Community, One Hope” culture through participating in quarterly culture initiatives and implementing teambuilding activities or creating other opportunities that promote a positive workplace culture. 
 
Minimum Qualifications
  • B.A. in Human Services related field (social work, early childhood education, family studies, psychology) or equivalent professional experience required. 
  • For Bachelor’s of Social Work degree or Master’s of Social Work degree holders, the appropriate DC license must be obtained prior to start date. 
  • Experience with providing public health case management and education (i.e., reproductive health, smoking cessation, child development, and parenting) to at risk populations preferably in the clients home setting strongly preferred 
  • At least 1 year of supervisory experience preferred. 
  • At least 2 years of experience in public health and/or human services field required. 
  • Demonstrated cultural competence in communicating with underserved, diverse populations. 
  • Strong written and verbal communication skills. 
  • Ability to work with computers and electronic health records. 
  • Knowledge of community resources or the ability to become knowledgeable required. 
  • Ability to establish and maintain trusting relationships with families. 
  • Strong interpersonal, organizational, analytical, problem solving and cross-cultural sensitivity skills required
  • Ability to work flexible hours including weekends and evenings required. 
  • Ability to conduct home visits. Valid driver’s license and vehicle required, as well as proof of auto insurance required. 
  • Proof of required vaccinations is required.  This includes, but may not be limited to, Flu and Covid. COH will consider requests for reasonable accommodations for anyone who cannot be vaccinated for a religious or medical reason, subject to applicable law.
