Job Responsibilities:
- Coordinating Home Healthcare services in office.
- Collaborating with healthcare providers, physicians, and different insurances.
- Developing and implementing individualized Plans of Care (POC) based on patient needs and family feedback.
- Supporting adherence to treatment plans and monitoring overall patient wellness.
- Recommending and coordinating community services as necessary.
- Collaborating with the primary doctor to ensure seamless plan implementation.
- Making adjustments to the Plan of Care as required.
- Completing all necessary documentation accurately and timely from the office.
Qualifications:
- Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN).
- Minimum of 1+ years of experience in case management within a Home Healthcare setting.
- Strong interpersonal and communication skills.
- Ability to coordinate and collaborate with a multidisciplinary healthcare team.
- Knowledge of regulatory requirements and best practices in Home Healthcare.
