Responsibilities:
- Provide high-quality, patient-centered care in the comfort of patients' homes
- Conduct comprehensive assessments and develop individualized care plans
- Administer medications, monitor vital signs, and manage patients' overall physical and mental well-being
- Collaborate with interdisciplinary healthcare teams to ensure coordinated and holistic care
- Educate patients and their families on disease management, treatments, and preventive measures
- Document and maintain accurate records of patients' progress and treatment plans
Qualifications:
- Active and unrestricted Registered Nurse (RN) license in the state of Florida
- Minimum of 1 year of nursing experience, preferably in Home Health or a related field
- Strong clinical skills and ability to provide comprehensive care to patients of all ages
- Excellent communication and interpersonal skills
- Ability to work independently and make sound clinical judgments
- Proficiency in electronic medical record (EMR) systems
- Valid driver's license and reliable transportation to travel to patients' homes
