Job description

Concordis Senior Living is looking for a dedicated and experienced Regional Clinical Specialist to join our support team based in Florida. We are an owner, operator, and developer of senior housing communities. Concordis operates an expanding portfolio of Senior Living communities throughout the southeastern United States.

POSITION SUMMARY

The Regional Clinical Specialist reports to the Vice President of Clinical and is responsible for ensuring the quality care of all residents by conducting site visits and audits of assigned communities. The Regional Clinical Specialist plays a pivotal role in ensuring the quality care of all residents within our communities. This includes conducting site visits and audits to uphold standards of excellence. Integral to this position is a deep understanding of, and commitment to, our company's mission and core values, as well as compliance with state regulations, company policies, and industry best practices. An essential aspect of the role involves working with the Community Care Teams and Resident population, requiring empathy and a genuine dedication to their well-being. Additionally, the Regional Clinical Specialist must possess strong delegation skills, balancing the assignment of tasks with maintaining overall responsibility and accountability. The ability to identify problems swiftly, develop effective solutions, and ensure their implementation is paramount. Collaboration with the Vice President of Clinical is key to fostering a cohesive approach to resident care and organizational success.

PRIMARY DUTIES AND RESPONSIBILITIES

  • Partner with community-level Care Team to oversee/assist the delivery of care to Assisted Living/Memory Care residents, ensuring all emotional, physical, psychological, and safety needs are met through program services and activities.
  • Collaborate with residents, families, and the Community to ensure appropriate levels of care for residents.
  • Develop and/or monitor documentation systems.
  • Prepare internal Ad Hoc reports as needed.
  • Correspond, coordinate, and meet with the Management Team as necessary.
  • Ensure compliance with all state regulations and maintain the highest standards of care.
  • Continually evaluate clinical policies, procedures, and programming.
  • Report on the effectiveness of all clinical initiatives.
  • Strive to maintain industry-leading care and memory care programming and services; evaluate new techniques and products for potential implementation.
  • Conduct site visits to audit, inspect, train, and develop site-level evaluations to enhance communities’ efforts in providing the highest quality of care.
  • On-site auditing of Carts, Charts, and Policy and Procedure adherence.
  • On-site coaching/training/in-servicing on charting, med pass, using computer applications, policy and procedure, and best practices.
  • Assist with onboarding processes for new team members.
  • LPN with 3-5 years of experience in Assisted Living.
  • Proficient computer skills.
  • Road Warrior willing to travel 75-100% of the time.
  • Scheduling experience.
  • Management experience.
  • Understanding of state regulations.
  • Strong problem-solving skills.
  • Participate in all regulatory surveys and work with communities to eliminate deficiencies

· FL CORE Administrator Certification

· Experience with Eldermark and/or Yardi

· Handle sensitive and confidential information with discretion.

· Health Care and/or senior living industry work experience

· Excellent verbal and written communication skills

· Strong interpersonal skills

· Possess sound judgment and reasoning abilities.

· Strong attention to detail and problem-solving skills

· Exhibits a strong drive for results and success

· Possess strong time management and organizational skills

· Maintains courteous, professional, and effective working relationships with employees at all levels of the organization.

· Remains calm under pressure

· Self-motivated with the ability to work independently

· Have a passion for Seniors and has the ability to build meaningful relationships

· Ability to follow Nursing process and company policies

· Ability to succeed in a dynamic, fast-paced environment

· Ability to manage multiple priorities effectively and meet deadlines

· Day/Overnight Travel as needed

· Team oriented

· Proficient in MS Office

Must possess high energy/enthusiasm, be a self-starter, have excellent time management skills, a strong initiative, and superior verbal/public speaking.
