!! URGENT NEED FOR A HOME HEALTH RN IN ALBUQUERQUE, NM !!

A facility in Albuquerque, NM is looking for Home Health Registered Nurse. If you are a skilled RN looking for an opportunity to contribute to a dynamic healthcare team, we invite you to apply.

REQUIREMENTS:

• Valid NM State RN License
• Home Health experience is a must
• Homecare Homebase EMR experience preferred
• OASIS documentation experience preferred.
• Must have a valid driver's license and a reliable vehicle.

ASSIGNMENT DETAILS:

• 4-30 days paid training
• 6-8 patient visits per day. (Within a 10 mile radius or more)
• Geriatric patient population.
• Samsung Galaxy tablets will be available.
• Pay will be per visit, depending on the visit.

PAY WILL BE PER VISIT TYPE + Mileage pay

DUTIES & RESPONSIBILITIES:

• Providing initial and ongoing assessment to determine level of functioning, including OASIS Assessment at appropriate time points.
• A physical therapist gathers background data, reviews referrals, selects and modifies evaluation tools, and evaluates patients determining the level of patient’s physical and psychosocial functioning.
• Developing and revising the plan of care in consultation with the physician and other care-team members.
• Responsible for planning, evaluation, and treatment implementation of assigned caseload.
• Preparing clinical progress notes.
• Supervising therapy assistants and Home Health aides as appropriate.
• Evaluating outcomes.
• Educate patients and/or families about proper safe use and recommend methods for installation of adaptive equipment.
• Plans and prepares written treatment plan based on evaluation of patient data
• Provide in-home treatment programs to assist patients in restoring function, relieving pain, and preventing disability
• Allow patients and their families to maintain dignity and independence while receiving safe, effective rehabilitation in the security of their home
• Consult with the physician and an interdisciplinary team in the development of a plan of treatment
• Drive to your patients’ homes throughout your assigned region on a daily basis
• Consult with the physician and interdisciplinary team in the development of a plan of treatment.
